The Omega site has the 6x36 blanket but it's an 8 week wait minimum.
They also sell the 8x30 blanket, in stock, $95.00. Would that work for this type of setup?

Re: Heat blamket question
I am not sure how wide your form is but the extra width could get in the way of your i bolts when you go to clamp down the bent sides.

Re: Heat blamket question
Also if the blanket is not in contact with anything it can over heat. So the part of the blanket that is over hanging could get very hot and be damaged. Also I think it would be to short.
I bought this one and am very happy with it. Pay an extra $31 shipped with UPS and have it in 5 to 8 days. Mine came even faster.

Thanks Jim, looks like the best alternative. I wasn't sure of the vendor, but if you've had a good experience that's encouraging.

Measure the length that is needed, holding in mind that there is little to no bending being done at the ends of the side material. I found that a 30-inch-long Keenovo blanket worked well for my use. Less expensive to buy, less clumsy in use (for me).
